Broker/Third Party Online Access Form (Key User) 
Use this form to authorize key user access, on behalf of your group, for a broker, consultant or third party administrator.

About Key Users: Key users are responsible for granting access to other individuals within your organization. The key user is also responsible for maintaining the listing of all users for the group including additions, terminations and/or modifications. Only an authorized key user assigned by the group administrator will have the ability to grant access to other individuals within your organization.
